For your rebate submission, please send a photocopy of the entire cash register receipt indicating the date of purchase, name of retailer, and model number of your Lexmark printer. For bundle rebates, the cash register receipt, invoice or packing slip must also indicate the price and model number of the PC purchased.

How do I know if the rebate has been extended?
If a rebate has been extended, a new rebate form is printable from the Lexmark Website. The expiration date printed on the rebate form will be enforced.
What happens if I do not receive my rebate check after 8 weeks?
We request that you wait a full 8 weeks before contacting us concerning your rebate submission in order to allow sufficient processing time. However, if you have not received a response after 8 weeks, then please notify us in writing at the address listed on the rebate certificate.
What if I do not cash my check prior to the deadline printed on the rebate certificate form?
The check must be cashed prior to the date specified on the check. Failure to do so will be deemed a rejection of the offer and will void the rebate.
